The HOP will provide vital care and support for patients with a life limiting illness in their communities.  This service is directly responding to patients needs and caring for patients as close to home as possible to enable them to continue living as normal and independent lives as possible. 

The HOP Vehicle will be staffed by a team of clinical nurses and will travel to venues across the Braintree District.  First and foremost it will stop at GPs' surgeries to provide a range of outpatient appointments providing symptom control and treatments as well as advice and referral to other services.  It will also be based near village halls and similar venues to involve local groups in a social and supportive service.  The flexible desgin of the HOP means that it can also be used for education sessions and patient group activities.
